Closure of Oroantral Communication with a Palatal Flap: A Case Report

Oroantral communication (OAC) is a frequent complication following maxillary molar extraction or sinus-related surgical procedures, which, if untreated, may evolve into a chronic fistula. This clinical report describes the successful closure of a persistent OAC using a palatal rotation-advancement flap. The technique provided a well-vascularized, keratinized tissue with minimal donor-site morbidity and maintenance of vestibular depth, allowing long-term functional and esthetic stability. Postoperative follow-up of this clinical case demonstrated complete healing and absence of sinus complications, confirming the palatal flap as a predictable and conservative alternative for oroantral closure in posterior maxillary defects.
